On April 22, 2022, MMed, Dr. Ho Cao Vu, who was working at Cho Ray Hospital, specializing in Plastic Surgery and Benign Disease Surgery, had an operation to remove the capsule and placed a new implant with a Harmonic scalpel for a 33-year-old woman who had grade 3 capsular contracture after 6 months of having an axillary breast augmentation.
As an expert in aesthetic surgery, Dr. Ho said these cases have a very high risk of recurrence of the capsular contracture after the surgery which removes the capsule to reset the implant. Therefore, it is crucial to choose a safe plastic surgery method from the beginning, as a result, limiting the possible risks.
She had an axillary breast augmentation at a beauty center in Ho Chi Minh City, after 1 to 2 months of pain symptom, she re-examined and the plastic surgeon said it was a normal symptom. In 5th month after the surgery, there was a state of shrinkage, deformity in some positions, the chest was deflected upwards. In the 6th month, the doctor said that she had capsular contracture and would undergo the surgery again. However, she decided to go to MMed, Dr. Ho Cao Vu, who has spent over 10 years of plastic surgery to examine, was asked by a doctor for an intensive magnetic resonance imaging (MRI) of the breast to assess the condition of mammary gland pathology, capsular contracture condition and implant diseases. The operation at Nam Sai Gon International Hospital on April 22, 2022 was performed in 3 hours.

Capsular dissection is difficult to perform
The doctor said that the client also has sclerosis in a number of muscle areas around the capsule, especially the lower chest area (before surgery: the polar area under the breast pocket is retracted upwards, creating disproportionate distance from the nipple to the breast line and from the nipple to the superior border of the implant), hence, capsular dissection is very difficult. After the capsular dissection surgery, enough capsular muscle areas must be cut to adjust the tissue in front of the implant to be softened on both sides. The right pocket chest cavity created in the first time was narrowed with insufficient area for the new 325cc breast implant (old 325cc implant) so the cavity was re-created. The operation required a lot of capsule being removed, despite stopping the bleeding instantly, the doctor still had to insert additional drainage tubes to let the secretions out within the period of 48 hours.
The steps of Capsulectomy surgery
– Giving general anesthesia for the patient.
– Removing old breast implants.
– Using Harmonic Scalpel to remove the capsule and capsular muscle areas.
– Using Harmonic Scalpel to create a chest cavity that is suitable for new breast implants.
– Placing the 325cc Mentor bag
– Suturing the incision and inserting the drainage tube for 24 hours.
– The patient is discharged from the hospital instantly after the surgery.
According to the doctor, some symptoms of capsular contracture after breast augmentation are tightness in the breast, which may begin to appear as early as a few months after your breast augmentation surgery, however, the operation which removes the capsule to reset the implant should be performed in 6th month after the surgery. Using Harmonic Scalpel in capsular dissection surgery helping the doctor perform accurately and stop the bleeding instantly. MMed, Dr. Ho Cao Vu said breast augmentation by BLUNT DISSECTION is an invasive method.

The process of using conventional knifes may engender high risk of bleeding, tissue damage, discharging abundantly in chest cavity, thus, there is a need to insert the drainage tubes after the surgery to limit the infection, capsular contracture and follow the postoperative bleeding. The method of using a Harmonic scalpel in combination with the point dissecting technique which is a less invasive method during tissue dissection, tissue release by direct observation of normal eyes or through the endoscopic screen system, can control the bleeding, limit the discharge almost completely with a high degree of accuracy during cavity formation, rapid healing after the surgery and minimize complications.
